Ozempic Constipation: Why GLP-1 Medications Back You Up and How to Get Relief

Ozempic Constipation: Why GLP-1 Medications Back You Up and How to Get Relief

By Dr. Onikepe Adegbola, MD PhD — Johns Hopkins-trained physician-scientist and founder of Casa de Sante

Key Takeaways

  • Constipation is the 2nd most common GLP-1 side effect (after nausea), affecting 12-25% of patients
  • Caused by delayed gut motility — the same mechanism that reduces appetite also slows the colon
  • Dehydration from reduced food and fluid intake makes it worse
  • It is treatable with hydration, fiber, magnesium, movement, and targeted motility support
  • Do not use stimulant laxatives daily — they create dependency. Use gentle, non-habit-forming approaches instead.

Why GLP-1 Causes Constipation

GLP-1 receptor agonists (semaglutide/Ozempic/Wegovy, tirzepatide/Mounjaro/Zepbound) slow gastric emptying. This is part of how they work — slower stomach emptying = prolonged fullness = reduced appetite. But this motility slowdown extends beyond the stomach to the entire GI tract, including the colon.

Three mechanisms combine:

  1. Slower colonic transit: Stool moves through the colon more slowly, allowing more water to be reabsorbed. Stool becomes harder and dryer.
  2. Reduced food volume: Eating 30-50% less food means less bulk in the colon. Less bulk = less stimulation of the stretch receptors that trigger the urge to go.
  3. Reduced fluid intake: Many patients drink less because they feel full. Less water = harder stools.

The Fix: A Step-by-Step Approach

Step 1: Hydration (The Most Important Step)

Drink 64-80 oz of water daily. Not coffee (diuretic). Not diet soda. Water. Set a timer if needed — every hour, drink 8 oz. If you do nothing else on this list, increase your water intake. It is the single most impactful change.

Step 2: Fiber

  • Soluble fiber (psyllium husk, oats, chia seeds): Forms a gel that adds bulk AND moisture to stool. Start with 1 tsp psyllium in 8 oz water daily, increase gradually to 1 tbsp.
  • Insoluble fiber (vegetables, whole grains): Adds physical bulk. Important but can worsen bloating if added too quickly.
  • Start slowly: Adding too much fiber too fast will cause bloating and gas. Increase by 5g per week.

Step 3: Movement

Walking 20-30 minutes daily stimulates colonic motility through mechanical stimulation (the colon literally bounces during walking) and increased blood flow to the GI tract. Walking after meals is especially effective.

Step 4: Magnesium

Magnesium citrate or magnesium oxide draw water into the colon (osmotic effect), softening stool. 200-400mg at bedtime. This is non-habit-forming and provides the added benefit of improved sleep and muscle relaxation.

Step 5: Herbal Motility Support

When hydration, fiber, and magnesium are not enough:

  • Casa de Sante Regularity Companion — Herbal motility support formulated specifically for GLP-1 patients. Gentle, non-habit-forming, low FODMAP. Stimulates natural peristalsis without creating laxative dependency.

Step 6: Digestive Enzyme Support

Better digestion means better stool formation. Digestive enzymes help break food down more completely in the stomach and small intestine, ensuring optimal nutrient absorption AND normal stool consistency.

When to See Your Doctor

  • No bowel movement for 4+ days
  • Severe abdominal pain or bloating
  • Blood in stool
  • Vomiting combined with constipation (could indicate bowel obstruction — rare but serious)
  • Fecal impaction
