IBS Flare-Up Emergency Kit: What to Have Ready When Symptoms Strike
By Dr. Onikepe Adegbola, MD PhD — Johns Hopkins-trained physician-scientist and founder of Casa de Sante
Key Takeaways
- IBS flares are not a matter of "if" but "when." Even with perfect diet adherence, stress, illness, hormonal changes, travel, or unavoidable food exposures will trigger flares.
- Having a pre-assembled emergency kit eliminates the panic of scrambling for remedies during a flare — which itself worsens symptoms through stress activation.
- A good flare kit addresses all symptom types (pain, diarrhea, constipation, bloating, nausea) because IBS can present differently each time.
The Complete IBS Emergency Kit
For Abdominal Pain
- Peppermint oil capsules (enteric-coated): IBgard or generic. Relaxes smooth muscle in the gut within 15-30 minutes. Evidence-based for IBS pain (NNT of 4). Keep at least 20 capsules in your kit.
- Heating pad or heat wrap: Thermacare abdominal heat wraps are portable. Heat at 104°F applied to the abdomen reduces visceral pain as effectively as ibuprofen in some studies. Activates heat-sensitive pain receptors that block pain signals.
- Buscopan (hyoscine butylbromide): OTC antispasmodic in many countries. Prescription in the US (dicyclomine/Bentyl is the US equivalent — keep a few on hand if prescribed). Directly relaxes gut smooth muscle.
For Diarrhea
- Loperamide (Imodium): Slows gut motility and reduces fluid secretion. Take 2mg at first loose stool, then 2mg after each subsequent loose stool (max 16mg/day). Works within 1 hour.
- Electrolyte packets: Pedialyte packets, LMNT, or similar. Diarrhea depletes sodium, potassium, and water. Replace them.
- White rice: Keep instant rice packets in your kit. Bland, binding, easily tolerated. The go-to food during a diarrhea flare.
- Bananas: Binding, potassium replacement, gentle on the gut. The "B" in BRAT diet.
For Constipation
- Magnesium citrate liquid: A single bottle (10 oz) for acute constipation relief. Works within 30 min to 6 hours. Osmotic mechanism draws water into the colon.
- Glycerin suppositories: For when oral medications aren't fast enough. Stimulates rectal evacuation within 15-60 minutes.
- Warm water + lemon: Stimulates the gastrocolic reflex. Often enough for mild constipation.
For Bloating
- Simethicone (Gas-X): Breaks up gas bubbles in the gut. Safe, OTC, works within 30 minutes. Take 125-250mg after meals or during bloating episodes.
- Peppermint tea: Keep peppermint tea bags in your kit. The warmth + peppermint oil provides relief for both gas and spasm.
- Gentle movement: Walk for 10-15 minutes. Movement helps gas move through the GI tract. Yoga poses: knees-to-chest, supine twist, child's pose.
For Nausea
- Ginger chews or capsules: Keep ginger candy or 250mg capsules in your kit. Works on nausea from any cause.
- Acupressure bands: Sea-Band type wristbands that apply pressure to the P6 point. Evidence for nausea reduction (originally studied for motion sickness and pregnancy nausea).
For Anxiety/Stress During a Flare
- Breathing exercise card: Write down your go-to breathing technique (e.g., 4-7-8 breathing: inhale 4 seconds, hold 7, exhale 8). During a flare, you forget everything. Having it written down helps.
- Headphones + calming audio: Gut-directed hypnotherapy app (Nerva is evidence-based). Guided meditation. Nature sounds. Distracting the brain from gut sensations reduces symptom severity.
- A change of clothes: The psychological relief of having backup clothes cannot be overstated. Pack underwear, pants, and a plastic bag in your work bag or car. This reduces the anxiety of "what if" — which itself worsens IBS.
Travel Emergency Kit (Portable Version)
Keep a small pouch with the following in your bag at all times:
- Peppermint oil capsules (4-6)
- Loperamide (4 tablets)
- Simethicone (4-6 tablets)
- Ginger chews (4-6)
- Electrolyte packet (2)
- Digestive enzymes (10-12 capsules)
- Spare underwear + small plastic bag
During a Flare: The 3-Day Protocol
- Day 1: Restrict diet to BRAT + plain protein (chicken, fish). Increase fluids. Take enzymes with every meal. Rest. Apply heat as needed.
- Day 2: Continue bland diet. Add gentle cooked vegetables (carrots, zucchini). Reassess symptoms. If diarrhea: add loperamide. If constipation: add magnesium citrate.
- Day 3: Begin gradual reintroduction of regular low FODMAP diet. Monitor closely. If symptoms haven't improved after 3 days, contact your provider.
🛒 Your IBS Emergency Kit Foundation
- Digestive Enzymes — The cornerstone of any IBS emergency kit. During a flare, digestive function is compromised. Enzymes reduce the burden on an inflamed, hypersensitive gut by pre-digesting food before it reaches irritated intestinal tissue. Keep a bottle at home, one at work, and capsules in your travel kit.
- Regularity Companion — For constipation-predominant flares: gentle herbal motility support that works with your body's natural rhythms. Avoids the harsh urgency of stimulant laxatives while still getting things moving. Better for IBS-C than aggressive interventions that can swing you into diarrhea.
- FODMAP Enzymes + Probiotics — Post-flare recovery. Once the acute symptoms resolve, the probiotic component helps restore the microbiome that was disrupted by the flare. The FODMAP enzymes ease the transition back to normal eating by handling residual food sensitivities during the recovery period.
Medical Disclaimer: This article is for educational purposes only. If you experience bloody diarrhea, fever above 101°F, severe abdominal pain that doesn't resolve, or symptoms lasting more than 7 days, see your doctor. These may indicate a condition beyond IBS that requires medical evaluation. Dr. Adegbola is the founder of Casa de Sante.
