The Evidence

Why the Warning Exists

In preclinical studies, rats given semaglutide and liraglutide at doses 10-50x the human equivalent developed thyroid C-cell tumors (medullary thyroid carcinoma). Rats have abundant GLP-1 receptors on their thyroid C-cells. This triggered the mandatory black box warning.

Why Humans Are Different

  • Human thyroid C-cells have FAR fewer GLP-1 receptors than rodent C-cells
  • Primates (including humans) do not show C-cell hyperplasia with GLP-1 agonists
  • Large-scale human data (SUSTAIN, STEP, SURPASS trials + post-marketing surveillance): no signal of increased MTC
  • Calcitonin levels (a MTC marker) do not increase with GLP-1 use in humans

  • ✅ History of papillary thyroid cancer → generally OK (different cell type than MTC)
